Doctors often ignore Obesity and Overweight in the exam room

Wouldn't you think that a doctor would be very concerned with the weight of their patient? After all the very first thing you do when heading to the exam room is step on the scale!

I do not allow my doctor's office to weigh me if I am there about a sore throat, I tell them if I come in here looking as though I have gained some weight then by all means, weigh me, but I am a size 8 and therefore not obviously in need of weight management.

However, when I was 300+ pounds, my weight was the underlining issue with any problem I was frequently in there with! I was overweight, unhappy and very sickly. Well, no kidding ; my diet was made up of mostly fast food, soft drinks, ice cream and high fat "diet" bran muffins (bran sounds so healthy! I was fooled into thinking they were good for me!)--- little wonder why I was sick all the time. I was Obese but practically malnourished! Yet my doctor took my tonsils out, gave me antibiotics monthly but NEVER told me that the food I ate was keeping me sick and making me fat, he just said "diet and exercise" would help, but I assured him that I was "dieting" most of the time and getting some aerobics in weekly. My diet consisted of eating a bran muffin and diet coke for breakfast, starving all day and then fast food for dinner. I tried but failed.

When I quit "dieting" and starting living a more healthful lifestyle; cooking meals rather than relying on McDonald's horribly fattening foods, started drinking water rather than diet Coke, starting not only going to aerobics but actually pushing myself and doing more while there and going more often (and not rewarding myself for going with a trip to the bakery!)

NEW YORK - Few obese adults receive a formal weight-management plan from their doctors, despite the proven health benefits of even modest weight loss, a new study suggests.

The researchers reviewed the medical records of 9827 patients seen at the Mayo Clinic in Rochester, Minnesota, between November 2004 and October 2005. A total of 2543 of these patients were obese.

Principal investigator Dr. Warren G. Thompson, and his colleagues, found that only 505, or about one in five obese patients had their condition formally documented. However, patients who did have a formal diagnosis of obesity were 2.5 times more likely to be given a plan of treatment, such as diet changes and exercise goals.

Better management of obesity could have a “huge public health impact,” according to Thompson and his group. Even a modest weight loss, they point out, can help control a range of chronic health problems, such as high blood pressure, type 2 diabetes and high cholesterol.

It’s possible, Thompson told Reuters Health, that when doctors think about obesity as a medical problem that can be diagnosed, they are apt to discuss ways to manage the problem.

Not surprisingly, he noted, study patients who were severely obese were more likely to have a diagnosis and a treatment plan — suggesting that doctors need to take a closer look at patients with less severe weight problems.

Thompson also suggests that people who are significantly overweight and have yet to get advice from their doctors should consider broaching the topic themselves.

I think the reason is that doctors are overworked and underpaid and have to see as many patients as quickly as possible. They just don't have time to talk to them about anything other than they are 'seeing' them for on that visit--insurance companies frown on multiple diagnosis/codes in one visit!

Recently a local doctor referred his patient to me. I was surprised but he told her that he was stumped why she couldn't lose weight--test showed she was "normal" so he sent her to me. I had the opportunity to spend an hour talking with her, learn about her habits and her lifestyle.
